Recruitment Questions
Q: How often is the job list or job line
updated?
Usually once a week it is updated depending on the changes in
open positions.
Q: Can I email the recruiters?
Yes, their email addresses are as follows:

Nursing positions: Duarte.Mendonca@bethesdahealthcare.com
Q: Does Bethesda hire minors?
Yes, but the work hours are based on Child Labor Laws. We
need a parents consent and the applicant has to be at least 16
years old.
Q: Does Bethesda pay for relocation?
Yes, for hard to fill positions only and the applicant has to
have lived greater than 50 miles from the hospital. If those requirements are
met, relocation forms need to be submitted to HR with all moving receipts.
Q: Do I have to have a Florida RN License if I have an
out of state RN License?
Yes, you must have a FL RN License to practice as a RN.
Q: Do you hire Nurse Interns?
Yes, we hire nursing students who have completed at least 2
years of clinical as a Nurse Intern.
Q: Does a PCA have to have a FL certificate to
work?
Yes. Effective December 2005, PCA are required by
the Department of Health to have a certificate with the state of Florida.
Q: Do you have any opportunities for someone who wants
to get into Radiology?
Yes, we have an onsite Radiology school. Call ext. 4792
for more information.
Q: Do you offer any classes for nurses who want to
expand their nursing career?
Yes. At certain times of the year, we hold classes for
special areas such as CCU, Tele, etc to learn that level of nursing.
Benefit Questions
Q: Can you use direct deposit for any bank?
Yes. Once the direct deposit form and a voided check are
received in payroll it will take approximately 2 pay periods to go into effect.
Q: How do you cancel or change direct deposit?
If you want to stop direct deposit immediately go to
payroll. If you are going to enroll in direct deposit with another bank it
will take approximately 2 pay periods to go into effect.** Upon termination of
employment, the employees final wages will be paid by payroll check not direct
deposit and sent to Human Resources.
Q: How do I change my name and address?
You must provide this information to Human Resources. You must
notify all providers of change in address/name change. If you
are newly married, a copy of the marriage certificate is required. If
divorced, a copy of the divorce papers is needed.
Q: How do I change my Primary Care Physician?
Pick a new doctor from the provider directory and CIGNA Customer
Service: 1-800-244-6224.
Q: How do I enroll in my benefits?
Human Resources will send a notification letter to your home
address. This letter will let you know when your benefits will start and
what enrollment session you must attend to sign up for your benefits. If you
cannot attend the session you must call ext.4443 to set up an alternative
appointment.
Q: How do I sign up for Cobra?
Cobra is automatically done when an employee shows up on the
termination report. Ceridian will send Cobra packets to the employee's
home address via certified mail. Human Resources can provide rates to you, call
x4443.
Q; How do I sign up for the 403b/401k?
The Met Life Rep is at the hospital Tuesdays
and Wednesdays in the Human Resources Department.
Q: What happens if I lost my insurance card?
Call the insurance company CIGNA Customer Service 800-244-6224.
Q: What if my dependents have different last names then
the employee, what is required for insurance purposes?
Marriage certificate if the dependent is the spouse
Birth certificate(s) if the dependents are children
Q: Is a doctor's note required for FMLA and/or
disability?
Yes, the FMLA green doctor's certification needs to be completed
prior to the leave and a doctor's note is required to return to work. For
disability, a doctor's note is needed for going out and returning to work.
Q: When am I eligible for FMLA?
If you have worked for Bethesda for one year and worked 1,250
productive hours the year prior to the leave you are eligible. FMLA forms need
to be completed by a certified physician.
FMLA protects your job for a up to a 12 week period while
you are on leave.
Q: Where can I find the FMLA green forms that I need
completed by the physician?
For forms, you can go to HR Connection under the FORMS Tab,
or stop by HR for the FMLA green form. This needs to be complete in order for
you to use FMLA.

Q; When can I add, stop or make a change to my
benefits?
A qualifying event must take place or open enrollment.
Qualifying event is: marriage, divorce, birth or adoption
of a child, death, change in insurance coverage with spouse.
Q: When is open enrollment?
Open enrollment is held in November

Compensation Questions
Q: How is leave accrual computed?
Formula: Hours worked divided by 80 hours hours multiplied by
the employee accrual rate.
Example: 70 hours worked /80 * 7.5 = 6.56 accrued leave hours
Q: When is payday?
Bethesda's official payday is Friday and paper checks are
available Friday morning every two weeks in the Payroll Department. Employees
with direct deposit should have their funds available in their accounts on
Thursday afternoon.
Q: Where is the payroll office located?
It is in the executive wing of the hospital, next to the Clayton
Conference Center. They are open M-F 7:30 am to 4 pm.
