Benefits at Bethesda

Benefit deductions are taken
biweekly (24 or 26 pay periods) AND all benefits go into effect the first of the
month following 90 days of employment.
One Exception : 401k, 403b can go into effect
the first pay period following the election.
- Health Insurance -employees have a choice
to pick from a HMO & POS plan. If he/she is already covered under another
health insurance policy, he/she may be eligible to receive the Opt-Out monies.
Our health plans offer prescription drug benefits with low co-pays for both
formulary & generic.
- Dental Insurance - employees have a choice
to pick from a HMO & POS plan. Two options available: PPO & Managed
Care.
- Vision Insurance - covers all routine eye
care including eye exams and eyeglasses or contacts at low monthly
premiums.
- Flexible Spending Accounts - Health Care
Reimbursement and Dependent Care Reimbursement -accounts give employees the
opportunity to redirect monies on a pre-tax basis to pay dependent care and
health care expenses.
- OPT-OUT (Flex Credit) - Only applies to
current Bethesda insurance. Must show proof of insurance coverage
elsewhere. Medicaid not eligible for OPT-OUT.
- Basic Life Insurance $10,000 -A $10,000
Life Insurance policy is offered at no cost to the employee.
- Voluntary Life and Accident
Insurance
-additional insurance may also be purchased for your spouse and/or
dependent children at the below group rates.
- Accident Insurance -accident disability
income benefit. Individual or family coverage.
- Cancer/Specified DiseaseProtection
Plan -
provides medical insurance for cancer and other specific diseases.
- Long Term Disability
Insurance - provides
income protection against accidents or illness and may be purchased at low
group rates.
- Critical Life Crisis - is a revolutionary
new policy designed to help survivors of a critical illness.
- Long Term Care - insurance helps
people of all ages preserve their financial security and independence in the
event of extended disability.
- Life Insurance- Allows employees to take
out additional Life Insurance for themselves, spouse and dependents.
- Legal Plan -provides its members with legal
protection and peace of mind with predetermined, discounted fees on all types
of legal services.
- 401K- employees hired after 10/1/2005 eligible
for 401K w/ up to 2% matching.
- 403B - available for employees hired prior
to 10/1/05 and per diems and rehires (which varies based on vesting
schedule upon leaving).
- Credit Union - A full range of financial
services offered to members through the Gold Coast Credit Union.
- Referral Bonus -Cash awarded for
successful recruitment of "hard to fill" positions designated by the hospital.

- On Site Child Care Center @ Early Learning
Center - Educational/developmental care provided to children and
grandchildren of BMH employees age 6 weeks to 5 years. To find out more call
737-7733x 4060.
- Direct Deposit- is available with any bank
(including out of state banks) that has direct deposit capabilities.
- Disability Leave Bank - employees accrue
up to 4.0 a ppd in addition to their PTO. After one year of benefit eligible
time the employee can access this bank to use for a short-term disability.
- Employee Assistance Program - Center for
Family Services provides counseling sessions, crisis intervention and credit
counseling.
- Education Reimbursement Tuition -assistance
for approved courses up to $2,000 dollars per year, amortized over 12 months.
Available after one year of benefit eligible service.
- Education Resource Center - assist with
ongoing management, supervisory and staff training on a wide variety of
subjects and courses.
- Employee Hotline -To provide open
communication to BMH employees (ext.4511).
- Employee Management
Exchange(EME)
-Meetings held to create and maintain good employee/management relationships
and promote open communication between staff and management.
- Health Assessment Physical - check-up,
including PPD and/or chest x-ray and routine lab work. Routine lab work can
also be done annually in the employee’s birth month. PPD’s should be
done annually prior to the employees hire month.
- Leave Time(PTO) - Paid time off program; holidays and sick leave;
leave days increase after 3, 5, 10, 20 years. Twice a year, employees
may cash-in a portion of their leave.

- Cash in Lieu of Leave - request to
cash in leave hours can be twice a year. November and May.
Benefit eligible after 1 year of service.
- Medical Library - for use by employees for
medical research.
- Pre-Paid College - Payroll deduction for
prepaid college plans: Florida, or 529.
- Relocation Allowance - with administrative
approval, up to $2,000 reimbursed for hard to fill positions recruited from a
distance of 50 miles or more. Monies are amortized over a 20 month
period.
- Shift Differential and Weekend
Differential
- Additional pay to employees who are assigned to work evening and night
shifts based on selected positions only.
- Sick Child Care - free professional care
within the Pediatric Dept. for mildly ill children while BMH employed parent
is at work.
